Thank you for participating in WA PREP! We are excited to work with you in providing comprehensive sexual health education (CSHE) to young people across Washington. Below we outline the process that facilitators will follow based on age and location type. The process is different for ages and sites, because federal requirements for data collection vary based on program setting. Please read these instructions carefully and follow them as closely as possible. Click on the box that matches your program setting to see the process you’ll need to follow!

1. Elementary and Middle Schools (4th – 8th grade)

Before you teach your first class...
  1. Inform parents and caregivers about your CSHE curriculum at least 30 days before instruction begins. Parents and caregivers always have the option to opt out of CSHE.
  2. Read Survey and Evaluation Guide to get acquainted with WA PREP evaluation and survey collection.
  3. Prepare all evaluation materials.
    Make sure you have online copies of:
    • (#) of Entry surveys and Exit surveys. Tip: Make double-sided copies.
      Middle School Entry SurveyEnglish | Spanish
      Middle School Exit SurveyEnglish | Spanish
    • 1 attendance sheet (this will be provided by your training manager)
    • 1 staff perceptions survey
    • 1 *checklist to include with evaluation paperwork.
While you teach...
  1. For all young people, please have them read (or read to them) the first page of the entry survey to obtain participant assent. Participants always have the option to opt out of surveys. Elementary school students (5th graders) will take the middle school entry and exit surveys.
  2. Give middle school entry survey to all students before teaching the first lesson. Remind students: NO NAMES. Please make sure no students write identifiable information on the survey.
  3. Teach all lessons.
  4. Document attendance for each lesson, watch the attendance guide for more information.
  5. Give middle school exit survey to all students. Please make sure no students write identifiable information on the survey.
After teaching is completed…
  1. Complete the staff perceptions survey. You only need to complete 1 survey per implementation. For example, if 2 people are co-facilitating, they can complete 1 survey together. If your site teaches more than one curricula, please complete 1 survey per curriculum.
  2. Complete checklist and send all materials (e.g. middle school entry and exit surveys, attendance, and staff perceptions survey) to [email protected]

2. High Schools (9th - 12th grade)
